The Shankle Clinic has a step-by-step process to make an appointment. This will allow our doctors to address your medical concerns and make your appointments as productive as possible.
You may have 2 separate consultations that will be scheduled on different days.
If you would like to make an appointment, please contact us by phone, fax or e-mail. We will send you the forms that are necessary for you to complete prior to your first appointment.
If you wish to contact our office by telephone and need to leave a message, please make sure to leave your name and telephone number including the area code.

You will receive a packet in a mail containing the forms for you to complete prior to scheduling an appointment.
Please fill these forms out completely. Once you have filled the forms out, please return the packet to our office.
You will also need to complete the Medical Record Release Form(s) and send them to the appropriate physicians and/or other healthcare providers who can provide information relating to the condition that you are going to be evaluated for.

Once we receive all information requested, we will call you to make an appointment. Please give our office approximately 1 week to contact you.
Your first appointment will last about 60 minutes. We will review your paper work, laboratory results, imaging results and medical records provided by your physicians.
You will be given a brief test of memory and other abilities, and asked questions about your medical conditions and primary concerns. ]
At the end of your first appointment, we will schedule your second consultation appointment.
Your second appointment will be 60 to 90 minutes. At this appointment, we will perform a more comprehensive set of memory and other cognitive tests, review all of your laboratory and imaging test results plus review the consultation report from the first appointment. Based on these results, a diagnosis will be made and a treatment plan developed.
Initially, while treatment is being adjusted, you may be seen every 1 to 2 months. Once treatment has been optimized, you will be followed every 3 to 6 months based on your condition and treatment goals.
