When you receive the packet containing the forms, please fill them out completely. Once you have filled the forms out, please return the packet to our office.
You will also need to complete the Medical Record Release Form(s) and send them to the appropriate physicians and/or other healthcare providers who can provide information relating to the condition that you are going to be evaluated for. Your records should be sent to our office:

A member of the Shankle Clinic staff will reach out to make an appointment once we receive the requested information. Please allow our office approximately one week to contact you. The patient’s first appointment will last approximately 60 minutes. During the initial visit, we will review the patient’s medical history, current concerns, laboratory results, imaging results, and the medical records provided by other physicians, if applicable. The patient will be given a brief test to check his or her memory and cognitive abilities. At the end of the first appointment, we will schedule the following appointment.
Cancellation and Missed Appointments: Due to the high volume of patients seen by the Shankle Clinic, patients must cancel or reschedule appointments at least 48 hours in advance of the original appointment. Patients who fail to do so may be charged a cancellation fee of $100.
Shankle Clinic is a participating provider for most major insurance carriers. Patients who plan to use insurance should contact our offices to verify that the patient’s plan is covered.
Patients enrolled in certain health maintenance organization (HMO) plans may require prior authorization from a primary care physician before scheduling an appointment with our clinic.
